Who is required to submit immunization records?
All New Students
All students entering K1 and K2 in the fall
All students entering 7th-grade in the fall

Who is required to submit a physical form?
Any student entering the first grade who has not submitted a completed physical form to the school within the past year

Am I required to submit an Immunization Form and/or Physical Form?
No. By law, you may elect to sign a waiver citing personal beliefs, but keep in mind, during an outbreak, only students who have received the appropriate immunizations will be allowed to attend school.

Waivers are located on page 2 of each form. Waivers must be signed and dated to be valid, and please note that there is both an immunization waiver and a TB waiver on the second page of the immunization form.
